Passport applications for minors often need a notarized Statement of Consent (Form DS-3053) when one parent cannot attend the appointment. Other passport-related affidavits and identity statements also need a notary.

I come to your home or office so the absent parent can sign without taking time off. The notarized form is then ready to submit with the application.

Pricing

Mobile appointments start at $20, including travel in the primary area and one notarized signature. Extra signatures, signers and mileage are quoted before the appointment.
